Health Connectors (“we”, “us”, “our”) operates an online introduction, discovery and matching platform that helps users discover and connect with independent healthcare providers. 2. What this policy covers
This policy explains what personal data we collect, why we collect it, the legal basis for processing, who we share it with, how long we keep it, and the rights you have. It applies to people seeking care (“users”) and to health professionals who join our network.
3. What data we collect
a. Information you provide directly
- Identity and contact details: name, email, phone, and similar.
- Intake information: what you are looking for support with, your preferences and search criteria (support category, location, language, availability, provider type, budget or insurance preference, practical requirements, and any preferences you choose to share).
- Health-related information you voluntarily choose to include in your intake or messages (see Section 4 — this is special-category data).
- Communications you send us (messages, contact-form submissions, feedback, reviews).
b. Provider information (for health professionals who join)
- Identity, address, phone, email and bank account details; KvK registration; professional registrations; and details of services, methods and experience — collected for verification, screening and integration into the matching tool.
c. Information collected automatically
- Technical and usage data: IP address, device and browser type, pages visited, and similar analytics data, collected via cookies and similar technologies (see our Cookie Notice).
4. Health-related information (special-category data)
Some information you choose to share may relate to your health. Under GDPR Article 9, health data is special-category personal data requiring extra protection. We process it only where you have given explicit consent, and only for the purpose of helping you discover and connect with potentially relevant independent providers. You are never required to share health information; you decide what to include. We do not use this information to diagnose, advise, or assess medical suitability — our tool structures the information you provide into search criteria only.
You may withdraw your consent at any time (see Section 8). Withdrawal does not affect processing carried out before withdrawal, and may be subject to legal and technical limitations.
5. Who receives your data
- Independent professionals you choose to connect with: when you decide to contact or be introduced to a professional, relevant information you have provided is shared to enable that connection. Professionals are independent controllers of the data they receive and are responsible for their own handling of it.
- Service providers / processors: hosting, IT, analytics and communication vendors who process data on our behalf under data-processing agreements. [List categories / key processors].

We do not sell your personal data. We do not share health-related information with anyone other than the professional(s) you choose to connect with, our processors, or where legally required.
6. International transfers

7. How long we keep your data

9. Security
